Transportation (TR1) – Mankisha Health Services LLC

Program Description

Mankisha Health Services LLC provides Transportation (TR1) services under the Arizona Division of Developmental Disabilities (DDD). This service ensures that members have safe, reliable, and accessible transportation to and from authorized services and community activities as outlined in their Individual Support Plan (ISP). Transportation supports community integration by connecting members to employment, day programs, medical appointments, habilitation services, and other essential activities.


Service Model

  • Service Scope: Transportation is provided only to and from DDD-authorized services and activities.
  • Vehicles: All company vehicles are well-maintained, insured, and comply with Arizona safety regulations. Vehicles are equipped to meet the accessibility needs of members, including wheelchair access when required.
  • Staffing: Drivers are trained in first aid, CPR, member rights, and emergency procedures, and must maintain a valid Arizona driver’s license with a clean driving record.
  • Scheduling: Transportation is coordinated in advance, per the member’s ISP and service schedule, with flexibility to meet individual needs.

Member Support Areas

  • Access to Services: Reliable transportation to employment, habilitation, medical, and therapy appointments.
  • Community Inclusion: Opportunities to attend community, recreational, or social activities when approved by the ISP.
  • Safety & Comfort: Members receive assistance with entering/exiting vehicles, seatbelts, securing mobility devices, and safe travel.
  • Consistency: Transportation schedules align with work, supported living, and day program hours to ensure continuity of care.

Mankisha Staff Responsibilities

  • Provide safe, timely, and respectful transportation services.
  • Assist members in boarding, exiting, and securing themselves or mobility devices in vehicles.
  • Maintain communication with program staff, families, and support coordinators to coordinate schedules.
  • Ensure vehicles are inspected daily for safety and cleanliness.
  • Document trips, mileage, and any incidents in accordance with DDD requirements.
  • Uphold member dignity, privacy, and rights during transport.
