Supported Living (HA1) – Mankisha Health Services LLC
Program Description
Mankisha Health Services LLC provides Supported Living (HA1) services under the Arizona Division of Developmental Disabilities (DDD). This program offers individualized support for members who live in their own home or apartment and need assistance to live as independently as possible in the community. Services are flexible, person-centered, and designed to promote autonomy, safety, and quality of life.
Service Model
- Setting: Member’s own residence (apartment, rented home, or family-owned home).
- Support Provided: Based on the member’s Individual Support Plan (ISP) and may include personal care, skill-building, health and safety monitoring, and community integration.
- Staffing: Direct Support Professionals (DSPs) trained to provide daily living assistance, behavioral supports, and emergency response.
- Schedule: Flexible support hours determined by the member’s needs (ranging from a few hours daily to 24/7 care).
Member Support Areas
- Daily Living Skills: Cooking, cleaning, laundry, grocery shopping, personal hygiene.
- Health & Safety: Medication reminders, monitoring for health concerns, emergency planning.
- Community Integration: Transportation and participation in work, social, or recreational activities.
- Money Management: Budgeting support, bill payment assistance, responsible financial decision-making.
- Self-Advocacy: Encouraging choice-making, independence, and participation in planning services.
Mankisha Staff Responsibilities
- Provide direct, respectful support in the member’s home and community.
- Teach and reinforce skills to promote independence.
- Ensure member safety while encouraging personal choice and responsibility.
- Assist with medical needs (per ISP), including coordinating appointments and following health care directives.
- Support community participation, including work, volunteering, and recreation.
- Maintain accurate documentation of services, progress, and incidents in compliance with DDD standards.
- Collaborate with DDD Support Coordinators, families, and health professionals to update and carry out the ISP.
